- Your driver’s license or a form of photo I.D.
- List of current medications, including dosage and schedule
- Completed New Patient Forms (see below)
- Records from previous doctors about your current foot condition

If you'd like, fill out our New Patient Information Packet before you arrive. Click the Patient Forms icon below to download and print the forms that need to be completed, signed and brought with you on your first visit.
Also, please remember to bring a list of any current medications you're taking including the dosage and schedule.
Podiatry Appointment Payments
All co-pays, deductibles and co-insurance amounts are due at the time of service. We accept Cash, Check, Visa & MasterCard.
