To set an appointment just give us a call at 205-445-0661. If you’re unable to call or if it is after hours we can also communicate via e-mail. Simply click the Request an Appointment icon, fill out the form and we will respond to your request by phone or e-mail.
NOTE: After clicking the icon you will enter our Secure Medical Web Office. You will be required to agree with our office privacy terms and conditions of use before entering.
What to bring on your first visit
- Your insurance card
- Your driver’s license or a form of photo I.D.
- List of current medications
- Completed New Patient Forms (see below)
- Records from previous doctors about your current foot condition
We want your first visit to be a good one, that's why Foot Specialists of Birmingham wants you to know what to expect.
New Patient Forms
We're giving you an opportunity to take your time to fill out our New Patient Information Packet before you arrive. Click the Patient Forms icon to download and print the forms that need to be completed, signed and brought with you on your first visit.
Also, please remember to bring a list of any current medications your taking including the dosage and schedule

We're sorry, but we do not accept Cigna or Medicaid
Payments
All co-pays, deductibles and co-insurance amounts are due at the time of service. We accept Cash, Check, Visa & Master Card. |
